Cortez-based C&G Health Care Management sells facility to ownership group with experience in skilled nursing facilities

The Vista Grande Inn skilled nursing facility in Cortez has changed ownership and has a new name.

An ownership group purchased the 101-bed nursing home from C&G Health Care Management on Sept. 1 and changed the name to Vista Grande Rehabilitation and Healthcare Center, according to a news release.

The facility sold for $10.29 million, according to data from the Montezuma County Assessor’s Office.

The new owner’s group, whose members were not identified, is made up of skilled nursing facility managers and operators based in Colorado and the Midwest.

“In these challenging times, it is very reassuring that our new ownership team has years of know-how, shared values and a great group of experienced experts – based right here in our region – to support our care team,” said Paula Mitchell, the center’s administrator.

The Four Corners Community Band has played for the residents at Vista Grande Inn.

Vista Grande Rehabilitation & Healthcare Center, 680 Hospital Drive in Cortez, will benefit from the hands-on attention that comes from a smaller, but very skilled “noncorporate” ownership and management team, Mitchell said in the release.

The team has overseen operations at similar care centers, in rural and suburban communities around Colorado and the Midwest, and brings many new ideas that have been very successful at other facilities.

“The ownership group has significant experience in overseeing operations at similar-sized care centers,” Mitchell said. “We anticipate incorporating new ideas that have been successful at their other facilities as we work to advance our mission of excellence in care.”

Vista Grande Rehabilitation & Healthcare Center is a 101-bed subacute and skilled nursing facility providing long-term care, short-term care, rehabilitation, occupational therapy, physical therapy, rehab-to-home, speech therapy, hospice care and respite care.

The center is accepting new admissions for short-term rehabilitation and long-term care. For further information, contact Kathleen Russell at (970) 564-1122 or visit the Vista Grande Rehabilitation & Healthcare Center website at www.vistagranderhc.com.
