Southwest Health System has launched its 2025 Community Health Needs Assessment survey to identify critical health issues in the region, and they are inviting residents and workers to provide feedback by May 23.
The survey aims to inform future collaborations, partnerships and community benefit planning. It is open to anyone living or working in the region.
The survey is available in English and Spanish. It is available in English online at www.surveymonkey.com/r/SHS2025CHNASurvey or in Spanish online at www.surveymonkey.com/r/EncuestaCHNAdeSHS2025. Responses will remain confidential.
